Free Medical Screening
Terms and Conditions:
- You must be a Malaysian Citizen
- Only one voucher per eligible person
- Your information will be kept confidential, and won’t be used for other purposes but ONLY for mailing the voucher to the qualified participants
- Consultation, medication and miscellaneous charges if applicable will be borne by the patients.
- Voucher is non-redeemable for cash and non-transferable.
- The name and IC No (New) of the patient entitled for the screening shall match the particulars printed in the voucher.
- The decisions made by Persatuan Diabetes Malaysia and its sponsor are final and non-negotiable.

PLEASE TAKE NOTE:
“Prior to your visit, please check with the respective doctor (as listed in this site) that you intend to consult, with regards to their charges for consultation / miscellaneous fees, as they may or may not impose a nominal fee for their professional service on top of the free medical screening”.
